Choose when you are looking for general mental health therapy for symptoms of anxiety, depression, grief and loss, substance use, anger management, or stress management. Also choose this intake form for mental health symptoms other than ADHD, Burnout, DBT Insomnia, or OCD.
Choose for intakes about ADHD screening assessment, ADHD therapy, parenting children with ADHD, or for ADHD services for your child.
Choose when you are a founder or executive and are dealing with symptoms of burnout and fatique. This NOT the right form for an executive Asseistant to complete.
Choose for services for DBT, Borderline, and Bipolar, or emotional regulation in general.
