About this notice
This Privacy Notice describes how STG Health Services Inc. (“STG Health,” “we,” “us”) collects, uses, stores, and protects personal and health information when you contact us, complete an intake, or receive services from our clinicians. Plain language is used throughout. If anything here is unclear, please contact our Privacy Officer at the address below — we want you to understand exactly how your information is handled before you decide to share it with us.
This notice covers our intake forms (including our online intake at intake.stghealthportal.com), our clinical services, our administrative communications, and our website.
Who we are and our role as trustee
STG Health Services Inc. is a Saskatchewan-incorporated psychotherapy practice based in La Ronge, providing telehealth counselling and psychotherapy services across the province. We serve rural, Northern, First Nations (under NIHB), and Métis (MN-SK) clients alongside our general practice.
Under The Health Information Protection Act of Saskatchewan (“HIPA”), STG Health is a trustee of your personal health information. This means we are legally responsible for protecting it, using it only for the purposes you have consented to, and following the requirements HIPA places on health information custodians.
Our Privacy Officer is Chris de Feijter (EdD, MACP, CCC, C-DBT), CEO and Clinical Lead. All privacy enquiries, requests for access or correction, and complaints can be directed to the Privacy Officer using the contact details at the end of this notice.
What information we collect
We collect only what we need to provide care safely and well. Depending on your interaction with us, this may include:
Identifying and administrative information
- Your legal name and date of birth
- Email address, phone number, and (where required) mailing address
- For status First Nations or Inuit clients: Treaty/Status Number to support NIHB prior-approval. We do not retain copies of your status card.
- For Métis MN-SK clients: MN-SK citizenship information where relevant to funding eligibility
Clinical and health information
- Brief screening responses (e.g. PHQ-9, GAD-7, ASRS, RIS-10, MSI-BPD, or other validated tools, depending on the area of intake you select)
- Your description of what is bringing you in and your goals for counselling
- Information you share in sessions, clinical notes, treatment plans, and any correspondence we send you
Information we may receive from others — only with your consent, for example a referral letter from your physician, a prior diagnostic report you choose to share, or correspondence with another clinician involved in your care.
We do not collect more than is needed. If you choose not to provide an optional piece of information, that does not affect your access to our services unless that information is required by a funder (e.g. NIHB).
Why we collect it and how we use it
Your personal health information is used to:
- Match you to the right service and clinician
- Provide clinical assessment, counselling, and follow-up care
- Communicate with you about your appointments and care
- Process insurance claims and direct billing where you have asked us to
- Meet legal and regulatory obligations (e.g. retention rules, mandatory reporting where applicable)
- Improve the quality of our services, in de-identified form only
We do not sell your information. We do not use clinical content for marketing. We do not share clinical content with advertising or third-party tracking systems on our website.
Who has access, and when we share
Within STG Health, only clinical staff bound by professional confidentiality and our internal privacy policy can access your record, and only for purposes connected to your care.
Outside STG Health, we share information only in these circumstances:
- With your express consent — for example, a letter to your physician, a report to an insurer, or a release to another clinician you are working with.
- For payment processing — funders such as NIHB, MN-SK, or private insurance companies receive the minimum information needed to process your claim. Clinical content is not shared with funders beyond the diagnostic descriptor and session record required for billing.
- Where required by law — for example, a court order, or a mandatory reporting obligation under Saskatchewan child protection law or where there is imminent risk of harm. We will tell you when this happens unless doing so would itself create risk.
- With service providers who help us operate — for example, our practice management software, our email service, our website host, and our encrypted intake platform. These providers are bound by data protection agreements and process your information on our behalf, never for their own purposes.
We do not transfer your health information outside Canada except where a service provider’s infrastructure is hosted in another jurisdiction (e.g. some Cloudflare, email, or AI-assistance services). Where this happens, we use providers with strong contractual and technical privacy protections and limit what information leaves Canadian-hosted systems.
How we protect your information
Protecting your information is a daily practice, not a one-time setup. Our safeguards include:
- Encryption at rest — all clinical intake responses and consultation letters are encrypted with AES-256-GCM before being stored. Encryption keys are derived from server secrets that are not shared with any third party.
- Encryption in transit — all communications with our portal, intake form, and clinical software are encrypted using TLS.
- Access controls — only authorised clinical staff can access your record, and access is logged. Each clinician has their own credentials; shared logins are not used.
- Multi-factor authentication on our administrative systems.
- Locked physical environment for the small amount of paper material we hold.
- Encrypted device storage on any laptop or mobile device that touches client information.
- Bot protection on our online intake (Cloudflare Turnstile) to reduce the risk of abusive automated submissions.
- Retention and disposal policies as described below.
No system can promise perfect security, but we work continuously to reduce risk and respond quickly to any incident.
How long we keep your information
We follow the Canadian Counselling and Psychotherapy Association (CCPA) Standards of Practice:
- Clinical records are retained for seven years after the last date of service.
- For clients who were minors at intake, records are retained for seven years after the age of majority.
- Where local policy or law requires a longer retention period, we follow that.
After the retention period ends, records are securely destroyed.
Administrative records that are not clinical (for example, billing summaries) may be retained for shorter or longer periods to meet tax, accounting, and regulatory obligations.
Your rights under HIPA
You have meaningful rights over the information we hold about you. You may:
- Ask what we hold about you and receive a copy of your record, subject to the limited exceptions HIPA allows.
- Ask for corrections if you believe something in your record is inaccurate or incomplete. Where a correction is not possible (for example, where the original clinical observation must be preserved), we will add a notation of your disagreement to the record.
- Withdraw consent for future contact at any time. If you withdraw consent during active care, we will discuss with you what that means for any care already underway.
- Receive a copy of your consultation letter when one is prepared for you. STG Health offers a secure DOB-gated letter portal so you can view and print your letter at your convenience.
- Make a complaint — directly to our Privacy Officer, and/or to the Saskatchewan Information and Privacy Commissioner.
To exercise any of these rights, contact our Privacy Officer using the details below. We will respond within 30 days where reasonably possible.
Cookies and our website
Our public website uses a small number of cookies needed for the site to function (for example, to remember accessibility preferences). We may use privacy-respecting analytics to understand how the site is used in aggregate. We do not use behavioural advertising trackers, and we do not sell or share visitor data with advertising networks.
Our online intake at intake.stghealthportal.com does not place behavioural tracking cookies. The intake uses Cloudflare Turnstile to reduce abusive automated traffic; Turnstile is designed to operate without behavioural profiling.
Children and minors
We provide services to clients of all ages where clinically appropriate. For clients under the age of majority in Saskatchewan, consent and information-sharing arrangements depend on the client’s age, capacity (including assessment as a “mature minor” where relevant), and the involvement of a parent or guardian. These will be discussed at intake.

We may update this notice from time to time to reflect changes in our practice, our service providers, or the law. The “Last updated” date at the top of this page indicates when changes were last made. Material changes will be highlighted in a notice on our website.
